Essential information to keep note of in case you need to recover your account:

  1. Player ID
  2. Player Master Nickname
  3. Player Master Level
  4. Number of Saint Quartz Owned
  5. Number of Rare Prism Owned
  6. Number of Unregistered Spirit Origins Owned
  7. Grailed Servants Info
  8. Notable Details of Your Servant Roster (Non-welfare Gold Servants owned in particular)
  9. Total Login Bonus
  10. Saint Quartz Purchases

Less essential but nice to have info:

  1. Friend’s Friend Codes
  2. Birthday
  3. Last Saint Quartz Usage
  4. Last Quest Played
  5. Device Model Name and/or Number

This guide was based off the JP’s guide, taking in account of the current NA environment.

Note: Please do not send the form to [email protected] , they will not handle any requests to recover accounts.

If there’s any information you are unsure/uncomfortable about, it’s safer to address that you “do not know/forgot” the information, instead of doing guesswork. Also you can also insert “did not set” in some applicable fields (for example your birthday).

Let’s start from the top, the boxes on the left are the questions, while on the right are notes.

Fields with * are highly recommended to be filled out.

Note: If you have associated an Apple or Google account to your former device, please send the email from those accounts. They have previously flagged emails from alternate addresses, it’s better to be safe on that regard.

What was your old account ID/Friend Code?You can check it at “MENU” > “Friend” > “Friend Search”.User ID is always a 9-digit numeric ID.
What was your latest transfer code?Please enter your last Transfer Number. You can add your other transfer numbers in Additional Information section.
What was your Master’s name?Master Name is the name displayed in “MENU“ > “My Room“ > “Master Profile“
What Birth Date did you set?If you haven’t set anything here, please write as “not set”.
What Device did you play on? (Please provide the Device manufacturer & model) The device model can be checked under “About Phone”, Model Number for Android.

When did you first download the game?This is the day where you have installed the application in your device that your last account was in, the day where you downloaded the game from Google Play Store or Apple Store.
When did you last access your old account?Please do specify the timezone (either PDT/EST or UTC+X). Ignore Time if you aren’t able to recall.
Which Servants did you possess?Aniplex requires 3 servants, however you’ll want to list the distinguishable part of your roster to make the process smoother. You CAN include your entire roster, but i’d recommend listing at least all your non-welfare golds. Please detail each of them as much as possible, in a concise manner. It’s up to you what you want to detail after writing down the servant’s name.

Who was registered as your Favourite Servant?If you haven’t favorited any servant, Mash is the default option.
Which Servants were in your main party?Strive to be as concise as possible, detailing as much as you are able to. This is the Latest Setup you have used.

What was your Master’s level?Master Level is under the Master Gender. 130 is the current level cap. If you are unsure, please give an estimate.
Do you remember the Master Name or Friend Code of any friend you had in your Friendlist?Please list as many friends as possible, each with the name and their User ID.
Did you ever purchase Saint Quartz?

For accounts that had purchased Saint Quartz, payment information is also required.Google Play Order number will be of format of:GPA.WWWW-XXXX-YYYY-ZZZZZ- If your purchase was made from an iOS device, please provide a screenshot of either your receipt or your iTunes Purchase History page. Please ensure that all of the relevant details (order ID, game name, date , and amount spent) are visible in your screenshot.

Maximum image size is 3MB. It’s highly recommended converting it to JPEG before uploading to reduce the file size.

Additional information that can help aiding in the account recovery, not part of the Recovery Form used by Support:

Please specify the latest singularity. If you want to, you can also specify the latest completed chapter of the current singularity.

Please specify the last quest you played.

Please enter the Total Login Bonus instead of Login Streak.
Example: “10th Day”, “50th Day”, “350th Day”, etc
Latest Saint Quartz Usage*[input]

Please specify when and how you used it.
Example: “10x Summon in the Story Banner”, “Restore Spirit Origin in SUN: Saber Training Ground – Advanced”, etc.
Saint Quartz Owned*[input]
Rare Prism Owned*[input]

Please enter the number of Rare Prism owned instead of Mana Prism.
Unregistered Spirit Origins Owned*[input]
Holy Grail Usage*[input]

Please specify how much and on which servants the grails are used.Example: “Altria Pendragon, Grailed to Lv.100 (5 grails).”

You can provide a brief explanation of the situation (Made a mistake during transferring devices, losing access to the device).
